Sexually transmitted infections (STIs) can occur without noticeable symptoms. This means a person may feel well while still having an infection that can be detected through laboratory testing. WHO supports targeted screening for certain STIs in people without symptoms, based on individual circumstances and the local setting.
If you are considering STI screening in Thailand, there is no single set of tests that is right for everyone. The most appropriate screening depends on factors such as previous testing, possible exposure, symptoms, sexual history, vaccination status and individual risk.
STI and HIV trends in Thailand
Syphilis, gonorrhoea and chlamydia are among the most commonly reported sexually transmitted infections in Thailand, and national surveillance has shown rising reported rates of major STIs in recent years, particularly among younger people.
HIV also remains an important public-health consideration. In 2024, Thailand’s Department of Disease Control estimated around 565,600 people were living with HIV and approximately 8,100 new infections occurred during the year, with about half of estimated new infections concentrated across 15 provinces, including Bangkok, Chonburi, Khon Kaen, Chiang Mai and Nakhon Ratchasima.
At the same time, Thailand has made major progress in its response to HIV. In 2016, it became the first country in Asia and the Pacific to receive WHO validation for eliminating mother-to-child transmission of both HIV and syphilis, and access to HIV treatment is now high. These achievements reflect decades of investment in prevention, testing and treatment, although continued screening and prevention remain important.
Geographic patterns do not necessarily reflect an individual’s personal risk. Risk depends more on factors such as sexual exposure, prevention practices and testing history than on location alone.
What can STI blood tests detect?
Blood testing is commonly used when screening for infections such as HIV and syphilis, as well as hepatitis B and hepatitis C where appropriate.
Different laboratory tests look for different markers. For example, syphilis screening may use an RPR test as part of the testing process, while HIV testing may detect antibodies, antigens or viral genetic material depending on the assay being used.
Hepatitis testing also needs some context. A hepatitis B surface antigen (HBsAg) test looks for evidence of current hepatitis B infection, while additional markers can provide information about immunity or previous exposure. Hepatitis C antibody testing identifies evidence of previous exposure, but further testing may be required to determine whether an active infection is present.
These distinctions are important because a screening result is not always the same as a definitive diagnosis.
Why are urine tests used?
Chlamydia and gonorrhoea are commonly detected using highly sensitive molecular tests known as nucleic acid amplification tests (NAATs), which include PCR-based methods. Urine samples can be used for urogenital testing for these infections.
The most appropriate sample depends on the site of possible exposure. In some circumstances, samples from other sites, such as the throat or rectum, may also be appropriate.
This is why a urine test should not necessarily be considered a complete STI screen in every situation.
Is a larger STI panel always better?
Not necessarily.
Modern multiplex PCR panels can detect many organisms from a single sample. These tests can be useful when broader investigation is appropriate, but the number of organisms included does not automatically make a panel more suitable for routine screening.
Some organisms found on broad genital-health panels can also be present without causing disease, and not every organism detected on these panels is conventionally classified as an STI.
International guidance generally supports risk-based and clinically appropriate testing, rather than assuming that everyone benefits from testing for every available organism.
Who should consider STI screening?
There is no universal screening schedule suitable for every person.
Testing may be worth considering if you have had a new partner, have had a possible exposure, have a partner who has been diagnosed with an STI, have not been tested for some time, or belong to a group for whom more frequent screening is recommended.
Screening recommendations vary between countries because infection rates, healthcare systems and public-health priorities differ. People with symptoms should seek appropriate medical assessment rather than relying only on routine STI screening.
When should you test after a possible exposure?
Timing is important.
If a possible HIV exposure occurred within the past 72 hours, seek urgent medical advice about post-exposure prophylaxis (PEP) rather than waiting for a screening test. PEP should be started as soon as possible.
STI tests have window periods: the period after infection during which a laboratory test may not yet reliably detect it. The length of this period varies according to the infection and the testing method used.
For HIV, for example, nucleic-acid testing can generally detect infection earlier than antigen/antibody or antibody-only testing. A negative result obtained very soon after a possible exposure may therefore need to be repeated later.
Rather than applying a single timing rule to every infection, the appropriate testing schedule should take into account the infection being investigated, the test method and the timing of the possible exposure.
Choosing an STI screening option
A useful STI screen is not necessarily the one containing the greatest number of tests. It is the one that provides appropriate coverage for your circumstances.
A blood-based screen can cover several important infections. Adding molecular testing for chlamydia and gonorrhoea provides broader screening for two common bacterial STIs. More extensive molecular panels and earlier-detection testing may be useful in selected circumstances.
